Athletic Clearance Process

To participate in NHS Athletics during the 2024-2025 School Year the athletic clearance will open May 25, 2024

 

Make Sure you have the following before you start your Athletic Clearance:

 

  1. Updated Physical (must have physician signature and date)

EL2 Form FHSAA EL2 Physical Form

  1. Students must complete NFHS courses (http://nfhslearn.com/ 
      1. Create or sign in to NFHSLearn account. Complete the following courses.
        1. Concussion in Sports course
        2. Heat Illness Prevention course
        3. Sudden Cardiac Arrest course
      1. Screenshot or Download certificate (certificate showing student name) for each course (certificate dates only accepted beginning 5/25/24)

Start Athletic Clearance

  1. Create an athletic Clearance account.
  1. Go to https://athleticclearance.fhsaahome.org/
  2. Click Add New Clearance, select the current year, and Northview High
  3. Sport: click the sport you desire

Note: Add all sports you plan to participate in during the school year

  1. Fill out all the information and upload documents
  1. Upload: EL2 form (physician signature and date), Concussion in Sports Certificate, Heat Illness Prevention Certificate, and Sudden Cardiac Arrest Certificate
  2. Notarized Drug Consent Form drug consent form
